How to Submit a Prescription
Submit a valid patient-specific prescription with the patient’s information, medication, strength, dosage form, directions, quantity, and refills as applicable
Prescriber Information Required
Please include prescriber name, practice information, NPI, phone number, and other information required for a valid prescription.
Prescription Processing & Support
Our pharmacy team reviews each prescription and will contact the prescriber if clarification or additional information is needed.
